Project GRACE is a BHCA outreach initiative providing
Medicaid and other health program eligibility assistance and
community-based healthcare linkages to Baltimore City substance
abuse treatment consumers. |
|
|
Each consumer that Project GRACE successfully
assists in obtaining benefits vacates a publicly funded slot
that can be utilized by someone uninsured.
As the program moves toward a 10% conversion rate, the
capacity growth in open treatment slots for the uninsured has
the impact of a new treatment program opening at a fraction of
the cost. |

In addition, Project GRACE identifies and links to
care consumers with
chronic diseases that can be fatal if left untreated.
These diseases, including HIV viral infections,
hepatitis, diabetes, and hypertension threaten the consumer’s
ability to remain in treatment if not monitored.
In addition, chronic healthcare issues becoming acute
from lack of care drastically drain public health resources.

The project is a collaboration between BHCA and Baltimore
Substance Abuse Systems.
Both are quasi- public bureaus of the Baltimore City Health
Department. BHCA
primarily provides education and advocacy for
HealthChoice enrollees.
Substance abuse treatment providers find the services so
valuable that they are featured in grant proposals. |

extend care and compassion.

Substance abuse treatment programs interested in Project
GRACE
services must be State certified and located in Baltimore City.
Preference is given to programs funded by Baltimore City
Substance Abuse Systems, Inc.
The screening process includes determining the client’s benefit
status and healthcare issues.
The Treatment Advocates then formulate an appropriate
progress plan and collaborate with the client on its
implementation.
